Mental Health Practitioner, Recovery WORK SCHEDULE 100% FTE

DEPARTMENT DESCRIPTION The goals of the program are to promote community stability by securing housing, benefits and culturally relevant treatment to meet the needs of those served, and to assist clients in developing meaningful and productive life roles. This position on the Recovery Services teams provides engagement, case-management, outreach and linkage as well as counseling services.
JOB RESPONSIBILITIES Provide initial screenings and psychiatric diagnostic evaluations for new clients seeking services
Provide mental health case management and counseling/ therapy services for an assigned caseload of clients
As part of a multidisciplinary treatment teamwork with patients and their social networks to assist in their recovery from mental illness
Assess patient needs and determine interventions
May assist clients obtain housing and funding as necessary to prevent inappropriate use of crisis services
Facilitate diversion from voluntary and involuntary psychiatric hospitalization when clinically appropriate
Participate in case reviews
Document patient contact in the medical record
Develop and implement patient care plans
Engage in triage services
REQUIREMENTS A Master’s Degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work
OR Bachelor’s Degree in nursing or a behavioral science AND three years’ of experience in the direct delivery of professional mental health care
Licensed as a Mental Health Counselor, Licensed Associate Clinical Social Worker
Licensed as an independent clinician preferred
Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration
