HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT has an outstanding opportunity for a
INPATIENT CLINICAL LEAD PHARMACIST (PHARMACIST 4) .
WORK SCHEDULE • Full-Time / 40 hours per week
• Rotating/variable shift
POSITION HIGHLIGHTS • Assist the Inpatient Clinical Manager with the implementation and evaluation of pharmacy services
PRIMARY JOB RESPONSIBILITIES • Development, implementation and evaluation of pharmacy to manage protocols/collaborative drug therapy agreements and new clinical initiatives
• Standardization of practice where appropriate across clinical services
• Optimization of clinical tools and order sets in EPIC
• Coordination of journal clubs and/or student presentations
• Residency project development and residency project support
• Actively resolve issues identified with clinical services and work with other departments to solve service-related issues when identified.
• Detect, document and report adverse drug reactions and medication errors. Assist in developing safe medication practices to prevent errors.
• Perform the duties of a Pharmacist 3
REQUIRED POSITION QUALIFICATIONS Three years experience in pharmacy practice. Months spent in the completion of an American Society of Health-System Pharmacists accredited residency training program may substitute month-for-month for the experience.
LEGAL REQUIREMENT State of Washington Pharmacist license.
