The Department of Rehabilitation Medicine has an outstanding opportunity for a Patient Care Coordinator to join their team.
The UW Medicine Long COVID Clinic, formerly know as the PCRRC, is a multidisciplinary hybrid clinic that provides care to patients who are still experiences symptoms after a COVID infection.
The clinic’s home department, Rehabilitation Medicine, is seeking candidates for a full-time Patient Care Coordinator within the Long COVID clinic. This position is primarily remote with occasional need for in-person meetings (ideally one day, weekly), community events, or other in-person events. Candidate must be able to provide a working space to reasonable assure patient confidentiality during care navigation appointments and communications.
The Patient Care Coordinator will support a multi-year funded grant from the Agency for Healthcare Research and Quality (AHRQ). This program is focused on improving access to multidisciplinary care for patients with Long COVID, with a special focus on community outreach to historically underserved populations (such as Latino, AI/AN, LGBTQ, and rural populations), as well as facilitating training opportunities for providers across the state of Washington.

DUTIES AND RESPONSIBILITIES
Administrative and Care Coordination Support
  • Provide administrative support in clinical care, research, and programs developed under the AHRQ grant.
  • Receive and carry our direct requests from clinic/grant leadership and providers.
  • Handle clinic care coordination including referral management, maintaining accurate and complete registration and patient demographic data, track multiple referral streams and patients records. May complete disability forms in preparation for physician review and signature.
  • Ensure the outcome of treatment recommendations for continued care is conveyed to referring physicians.
  • Receive patient complaints; obtain information and data from records and appropriate clinic staff; compose report for Health Service Manager to review and resolve.
  • Coordinates complex scheduling of procedures, diagnostic tests, consultations, and other health services including support for preauthorization and coordination with health service resources.
  • Manage phone calls, emails, faxes, and letter correspondence pertaining to patient and/or provider follow-up, continuity of care, and referral decisions.
  • Maintain scheduling procedures consistent with clinic and patient access goals. Verify patient benefit information for seamless transition within clinic-based services.
  • Utilize system resources, including computerized information systems and staff from ancillary departments to support development of processes to meet the needs of the serve and patient.
  • Serve as contact with internal stakeholders including providing information regarding program policies and activities and provide advice regarding UW Medicine system services.
  • Professional handle sensitive patient situations utilizing a non-judgmental approach that acknowledges the diversity and varying needs of community populations.
  • Identify and reduce barriers to accessing care for Long COVID clinic patients.
    Program Administration and Data Management Support
  • Actively participate and contribute to regular grant and clinic meetings and conference calls.
  • Respond to inquiries, internally and externally, regarding appointment and patient status and other care coordination program related metrics.
  • Contribute to routine data quality control measures to ensure program data is accurate and up to date.
  • Assist with data collection and database management utilizing tools such as REDCap, Epic, and other software.
  • Develop, provide, and implement process improvement feedback in collaboration with team members.
    MINIMUM REQUIREMENTS
  • A Bachelor’s Degree in a medically related or public relations field
  • Two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology OR
    equivalent education/experience.
    DESIRED QUALIFICATIONS
  • Experience working in a multidisciplinary clinic or other similar healthcare site.
  • Team oriented with ability to work independently and ability to be flexible and adapt to change (e.g., new projects, shifting priorities, emergency deadlines, etc.).
  • Experience utilizing technology to collaborative within a team/community (such as Microsoft Office/Teams and Discord) and applied experience with electronic health record software (such as Epic) and data management systems (such as REDCap).
  • Familiarity with the complexities of coordinating and managing care of chronic health conditions that require high medical usage, such as Long COVID. This may include experience working in a Rehabilitation Medicine (PM&R) environment such as a clinic or lab.
