The UW MEDICAL CENTER - MONTLAKE - OUTPATIENT PHARMACY has an outstanding opportunity for a PART-TIME AMBULATORY STAFF PHARMACIST (50% FTE, Day Shifts) . The Ambulatory Staff Pharmacist (Pharmacist 2) is responsible for performing professional pharmaceutical dispensing and clinical functions in the Ambulatory Pharmacies. This position provides drug information and education to patients and family members, providers, nurses and other health-care professionals, and provides pharmaceutical care to patients.
PRIMARY RESPONSIBILITIES:
• Conduct a thorough patient profile review to assess the appropriateness of the prescribed medication including monitoring for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in medical record as required.
• Promote rational, cost-effective drug therapy by promoting the use of formulary items and optimizing patient care through appropriate selection of treatment options.
• Direct pharmacy technicians in all areas of technical responsibilities.
• Provide drug information to patients, physicians, nurses, and other members of the health care team.
REQUIREMENTS:
One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
LEGAL REQUIREMENT: State of Washington Pharmacist license.
