HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT has an outstanding opportunity for a
FULL-TIME CLINICAL STAFF PHARMACIST - ICU - FLOAT (Rotating/Variable Shifts) .
This is a float position that will be assigned to either the Trauma ICU or Neurocritical Care ICU teams on a rotating basis, depending on department need. Strong candidates will have an advanced degree or residency, or 2 years of experience in clinically-oriented hospital pharmacy practice.
PRIMARY RESPONSIBILITIES: • Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed
• Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations
• Provide drug information and education to medical teams, nursing and dietician staff
• Provide patient counseling and education
REQUIREMENTS: State of Washington Pharmacist License and one year of experience in hospital pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.
LEGAL REQUIREMENT: State of Washington Pharmacist license.
