 Patient Services Specialist 2 - Panel Navigator.     WORK SCHEDULE    Full-time, 100% FTE (40 hours per week) 
   Monday - Friday, 8am-4:30pm 
   Position is located at Harborview Medical Center

   POSITION DESCRIPTION  
 The Panel Navigator is accountable for population health activities and tasks in the clinic which may include, but are not limited to the following: Enhance the patient experience through proactive engagement and outreach around wellness, preventative care and striving to improve the “global” patient experience. This role is accountable for population health activities working closely with the care team in the clinic. Examples of population health activities are to maintaining accurate panels, ensuring team-based panel management, gap closure, referring patients for care coordination, care management, addressing disparities in care for vulnerable populations and ensuring all patients’ needs are met. 
   PRIMARY RESPONSIBILITIES  
   Engage the patient in care optimization 
   Empower the patient with knowledge and access to support resources. 
   Improve the patient and family experience of care. 
   Participate in Patients Are First measures and associated Medical Home goals and objectives. 
   Referrals tracking to internal and external resources. 
   Contact relevant community agencies to increase knowledge of services available for patients and families and help eliminate barriers to care. 
   Educate per protocol. 
   Assist in the utilization of culturally relevant and language appropriate materials for target populations. 
   When indicated through population review, provide outreach and assistance with patients/families in the coordination of various wellness and chronic disease activities in collaboration with the health care team. 
   Other Duties as Requested     REQUIRED POSITION QUALIFICATIONS    High school graduation or equivalent AND 
   One year of healthcare, administrative, or customer service experience 
  OR  
   An equivalent combination of education/experience
