The HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT has an outstanding opportunity for a FULL-TIME CLINICAL PHARMACIST - AMBULATORY PHARMACY CLINICAL SERVICES - CARDIOLOGY (Rotating/Variable Shifts) . The Clinical Pharmacist (Pharmacist 3) will provide pharmaceutical care to patients in the community and HMC Heart Institute.
Strong candidates will completed their PGY 1 and have at least 24 months of experience providing direct patient care in a clinical setting. Completion of PGY 2 residency in ambulatory pharmacy practice is preferred.
PRIMARY RESPONSIBILITIES:
• Collaborate with Outreach Team and clinic staff to individualize drug therapies promoting safe, effective and economical patient care.
• Identify barriers to medication adherence and create reasonable solutions taking into account individual factors such as language, cultural/religious practices, socioeconomic status, and physical/cognitive/emotional barriers
• Assume responsibility for monitoring and modifying the drug regimen when referred, to include the ordering and interpretation of laboratory testing.
• Provide refill authorization in conjunction with assessment of adherence, disease states, drug interactions, efficacy, and safety.
• Act as a drug information resource to Outreach Team members, clinic staff and patients.
• Identify patients with indications for drug therapy who are under-treated and optimize drug therapy.
REQUIREMENTS:
State of Washington Pharmacist License and one year of experience in hospital pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.
LEGAL REQUIREMENT: State of Washington Pharmacist license.
