HARBORVIEW MEDICAL CENTER PHARMACY is looking for an outstanding
PER DIEM PHARMACIST 2 (Rotating/Variable Shifts) to join our Inpatient Pharmacy team! The Pharmacist 2 will perform and supervise dispensing functions in the Inpatient, IV, and Outpatient pharmacies.
Strong candidates will have completed an ASHP-accredited residency program or advanced degree.
PRIMARY RESPONSIBILITIES: • Fill prescriptions for medication and compound pharmaceutical preparations.
• Supervise the preparation and delivery of any pharmaceutical preparations kept as stock on the nursing units.
• Monitor medication and intravenous solution therapy for rational use.
• Conduct a thorough patient profile review to assess the appropriateness of the prescribed pharmaceutical including monitoring individual patient profiles for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in patient drug therapy.
• Provide drug information to patients, physicians, nurses, and allied healthcare providers.
REQUIREMENTS: One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
LEGAL REQUIREMENT: State of Washington Pharmacist license.
