Swedish Medical Center is looking for a Social Worker (1.0 FTE, Day Shift) to work in Swedish Cancer Institute Integrated Care at Swedish First Hill in Seattle, WA.

The Social Worker assumes a case load of 35-40 patients a month, which includes identification, assessment and intervention of patient / family needs including coordination of cancer support resources, referral to appropriate resources and emotional supportive counseling to patients and their families receiving their care at the Swedish Cancer Institute. Facilitates educational classes and support groups. In addition, participates as a social work practicum instructor for interns. Must be able to commute to multiple Swedish Medical Center sites and affiliate locations to provide oncology social work services.

In this position you will:

• Review the high-risk screening tool and identifies patients and their families in need of social work services. Conduct pre-assessment and triages referrals among social work team.
• Complete a psychosocial assessment to determine patient and family strengths and needs relative to living and coping optimally with diagnosis, treatment, follow up care and survivorship. Includes gathering data about: patient and family psychosocial strengths and coping skills; support systems / care giving needs; ethnic, spiritual and cultural influences; functional status; housing, financial, insurance, legal and transportation systems.
• Provide therapeutic interventions designed to address current and future problems as the patient’s medical and psychosocial needs evolve. Includes Advocacy, direct assistance and coordination of care giving, housing, financial, insurance, legal and transportation resources; emotionally supportive counseling that is brief and solution focused; provision of pre-admission, transfer and discharge consultation; provision of information and referral to SCI, community and other resources and programs; and case management services.
• Develop a case plan with patient and family based upon mutually agreed upon goals to enhance, maintain and promote optimal psychosocial functioning throughout the patient’s continuum of care. Participate in multidisciplinary care planning with patient’s health care team and the patient and their family. Communicate psychosocial progress to physicians and other multidisciplinary team members.
• Organize and facilitate support groups and educational forums as needed.

Qualifications:

Required qualifications for this position include:

• Master's Degree in Social Work from an accredited School
• Two (2) years post-graduate experience in providing services to clients who have chronic and acute illness in a health care setting, preferably oncology; or graduated social work interns through the Swedish Cancer Institute internship program

Preferred qualifications for this position include:

• Licensed Advanced Social Worker (LASW) with the Washington State Department of Health under Chapter 18.225 RCW
• Hospice or home-health experience to maximize psychosocial intervention in assisting patients with issues of disability, separation, loss and dying
• Support group leadership experience
