UW Medical Center Pharmacy has an outstanding opportunity for a FULL-TIME PHARMACIST 3 (Day Shifts) to work within the MAIL ORDER PHARMACY at the Eastside Specialty Center. This is a limited, fixed-duration appointment, expected to last 12 months. The Pharmacist 3 performs clinical pharmacy services to assigned specialty clinic patients. The pharmacist will work with the health care team to implement and monitor specialty medication therapies for the achievement of the desired therapeutic outcomes. The pharmacist will provide drug therapy consults, participate in treatment protocol development, provide staff, patient and family education and precept pharmacy students and residents. Serve as a drug information resource for health care professionals, patients, and their families. The pharmacist will work with the specialty pharmacy team to ensure patients have access to needed medications and will provide clinical support and documentation when required.
Responsibilities: :
Optimize pharmacotherapy:
• Collaborate with providers to individualize drug therapies promoting safe, effective and economical patient care.
• Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care.
• Assume responsibility for monitoring and modifying the drug regimen when referred, to include the ordering and interpretation of laboratory testing.
• Act as a drug information resource to providers, nursing staff and patients.
• Ensure patient access to specialty medications by assisting with providing required clinical information to insurers when needed.
• Identify patients with indications for drug therapy who are under-treated and work with the providers to optimize drug therapy.
• Review clinic prescribing patterns with clinic staff to design targeted education to promote consistency with UWP guidelines (or other nationally recognized guidelines when needed)
• Participate in the Specialty Pharmacy quality management program as part of the Specialty Pharmacy Committee.
Pharmacotherapy Education:
Patient:
• Identify barriers to adherence to medication regimen, incorporating patient characteristics i.e. language, culture/religious practices, physical, cognitive, emotional, pain/discomfort, socioeconomic status.
• Utilize state of the art tools in educating the patient on self-management of disease and drug therapy.
Clinic Staff:
• Provide pharmacotherapy presentations and in-services
• Provide drug information supported by evidence-based practice
Pharmacy Department:
• Participate/present at pharmacy clinical conferences, staff meetings, journal clubs, and clinic-based conferences.
• Provide continuing education to other pharmacy providers in area of expertise.
Pharmacy Students and Residents:
• Provide instruction for students, residents, and staff pharmacists regarding pharmaceutical care, distribution and drug information.
• Develop and maintain objectives, monitor progress, and assist with projects of pharmacy students and residents.
• Obtain clinical faculty appointment and maintain connection with School of Pharmacy for didactic and experiential training opportunities.
Pharmaceutical Dispensing activities:
• Prepare and dispense medications in clinic-based outpatient pharmacy
• Conduct a thorough patient profile review to assess the appropriateness of the prescribed pharmaceutical including monitoring individual patient profiles for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received. Document relevant information in the patient medical record.
• Evaluate all prescribers’ orders and verify correctness of entries in computerized patient profiles
• Interview patients, assess barriers to medication adherence and provide patient-centered education regarding their medications.
• Counsel patients on medications dispensed reviewing indication, proper administration, side effects, duplications, etc.
• Report and document adverse drug reactions.
• Comply with all pharmacy laws and regulations
• Provide continuity for daily operations and patient interactions
• Direct the work of technicians, interns, pharmacy students, and residents
• Maintain accurate records of controlled substances dispensed.
Research:
• Participate in collaborative research with the Schools of Pharmacy, Medicine, Nursing, and Public Health
• Participate in collaborative research with members of your clinical team.
Other:
• Maintain a strong clinical foundation based on primary literature and critical assessment of evidence based practice issues.
• Publish articles in professional journals and make presentations at the local, state, and national level that will be useful to other practitioners.
• Maintain effective communications with the other clinicians and administrative staff of the Department of Pharmacy and other departments in the Hospital.
• Promote the image of service and good will to patients, public, and medical center staff.
• Participate in professional pharmacy organizations.
Skills:
• Strong customer service orientation
• Demonstrated ability to effectively use computerization for efficient operations
• Knowledge and ability to conceptualize, plan and implement new technologies, programs and services
• Professional and effective verbal and written communication skills.
DESIRED:
• Two years of experience in pharmacy practice
• Months spent in completion of an American Society of Health Systems Pharmacists accredited residency training program can be substituted month-for-month for the required experience. REQUIREMENTS: State of Washington Pharmacist License AND two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
