UW Medical Center-Northwest - Wound and Ostomy Care has an outstanding opportunity for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) .
WORK SCHEDULE
  • Part time / 75% FTE
  • Day shifts
    The anticipated start date for this position is Summer 2025.
    OVERVIEW
    The Advanced Practice Provider (APP) is a member of a multi-disciplinary team that includes attending physicians, surgical residents, and medical students. This position provides services to UW Medicine patients and families and collaborates with other clinical teams, nurses, social workers, and therapists in the inpatient and outpatient setting within the Wound Care Center, clinical offices, and procedural areas. The APP will be responsible for providing services independently and collaboratively with appropriate privileges, including initial and ongoing assessment, diagnosis and treatment of all wounds, arterial and venous disease, infection, pressure ulcers, and ostomy care.
    PRIMARY JOB RESPONSIBILITIES
  • Provides timely patient and family assessment for wound and ostomy patients
  • Participates in the assessment, planning, implementation, and evaluation of wound and ostomy service interventions
  • Collaborates with the physicians, primary care providers, subspecialty providers, and outpatient therapists to ensure coordination and continuity of care across settings
  • Attends appropriate clinical meetings to serve as a resource for staff and providers
  • Facilitates collaboration with the Outpatient Clinics to ensure continuity of care
  • Provides patient and/or family with health education, counseling and referrals to other health professionals and community resources as appropriate
  • Participates as a key member of the interdisciplinary team through the development of collaborative and innovative practices, quality improvement, and evaluation of patient care outcomes
    REQUIREMENTS
  • Current and valid state licensure as an ARNP or PA
  • Graduate Degree from an accredited Nurse Practitioner Program or Graduate of an accredited PA Program
  • Nationally Certified ARNP or PA-C with DEA
  • Proven excellence working in an interdisciplinary collaborative team environment
  • Effective interpersonal skills, and clinical expertise in the care of patients with wound and ostomy conditions requiring inpatient services
