  • DEPARTMENT DESCRIPTION
    This program provides intensive case-management services to high utilizers of King County Jail as part of a multi-program and multidisciplinary team. The position will report to the Vital team supervisor and the HMHAS Clinical Supervisor and Manager. This position will provide assertive outreach, engagement and treatment services, to Vital clients as provide linkage and coordination with on-going services at HMHAS.
    JOB RESPONSIBILITIES
  • As part of a multidisciplinary team provide intensive behavioral health treatment, case management and advocacy services for Vital clients.
  • Work with patients and their social networks as appropriate
  • Assess patient needs and determine interventions
  • Facilitate diversion from voluntary and involuntary psychiatric hospitalization when clinically appropriate
  • Chart patient contact in the HMC medical record and the multidisciplinary record at Reach
  • Provide linkages to services from mental health, substance abuse, developmental disability and other behavior health services appropriate for
    patients discharged from the ED
  • Develop and implement patient care plans

  • REQUIREMENTS
  • A master’s degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work
  • OR bachelor’s degree in nursing or a behavioral science AND three years of experience in the direct delivery of professional mental health care.
  • Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
  • Licensed as a Mental Health Counselor, Social Worker, or Psychologist
