The UW MEDICAL CENTER - MONTLAKE PHARMACY DEPARTMENT is seeking a highly-motivated FULL-TIME CLINICAL PHARMACIST - SOLID ORGAN TRANSPLANT (Rotating/Variable shifts) to provide pharmaceutical care for our solid organ transplant services. Our Clinical Pharmacists are a vital part of the multidisciplinary patient care team. In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, precept pharmacy students and residents, and perform duties of a Pharmacist 2. The Clinical Pharmacist, Solid Organ Transplant will provide clinical services in both the inpatient and ambulatory clinic settings.
PRIMARY RESPONSIBILITIES:
• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
• Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders upon hospital admission and discharge, as well as during clinic visits.
• Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care between the inpatient and ambulatory settings.
• Monitor and modify patient medication therapy in collaboration with the medical team to achieve desired therapeutic outcomes in the ambulatory setting
• Provide primary preceptorship of pharmacy residents and student pharmacists.
Strong candidates will have prior experience in solid organ transplant and/or PGY2 residency in this area.
REQUIREMENTS:
Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
LEGAL REQUIREMENT: State of Washington Pharmacist License.
