Responsibilities

The Fontan Wellness APP will be part of building a new Fontan program in the Heart Center to serve our regional single ventricle patients. The program will be an ambulatory and inpatient consultation role. Seeking an ambitious NP or PA with the vision and follow through needed to be a regional resource for Fontan wellness. Minimum of 2-3 years of pediatric cardiac experience as an APP.

The Advanced Practice Provider works with existing care coordination resources and medical/surgical providers to assure program-specific safe, cost-effective, and high quality patient care transitions among settings. This provider evaluates the medical and surgical status of the patient, and collects historical and physical examination data to create patient and family centered plans for transition between care settings. Follow up care may include activities such as obtaining histories, performing physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and ordering treatments, in accordance with the patient’s overall plan of care. The APP directs and/or participates in medical case management and promotes collaborative practice among specialists and community providers, and contributes to organizational and departmental continuous performance and quality improvement projects. The Transitional Care APP supports SCH system improvements to meet departmental and organizational goals, and participates in education and research efforts for patients, families, and the health care team.

Requirements

Required Education/Experience:
- Master’s degree in nursing with a concentration in a pediatric or family nurse practitioner program OR physician assistant program
- Minimum of three (3) years of recent pediatric clinical experience in the nurse practitioner role or physician assistant role.
- Experience in case management serving complex families.
Required Credentials:
- Active, unencumbered registered nurse and advanced registered nurse practitioner OR Physician Assistant licensure in the State of Washington.
- Current national certification as a pediatric or family nurse practitioner or physician assistant..
- Active Drug Enforcement Authority registration.
- At time of hire: Current Basic Life Support (BLS) for Healthcare Providers is required for all positions. Pediatric Emergency Assessment, Recognition and Stabilization (PEARS), and Pediatric Advanced Life Support (PALS) will be accepted in lieu of BLS for Healthcare Providers.
- As a condition of employment: Current BLS for Health Care Providers is to be maintained at all times by all nurses. Note: current PALS and/or PEARS will be accepted in lieu of BLS, and must be maintained as current at all times. Employees are responsible for any costs (including time away from work) associated with obtaining PALS or PEARS certification if it is not a requirement of the position. (ACLS will not be accepted as a substitute to the requirements listed above.)
- Active clinical privileges as an Allied Health Provider, Seattle Children’s Hospital.
- Prior to start of employment, must obtain, and maintain in good standing, medical or allied health professional staff privileges that are not limited, revoked, suspended, conditioned, or probationary, except for normal probationary periods due to initial appointment. All medical and allied health professional staff members are provisional for the first year.
- Any and all medical or allied health professional staff privileges at Children's will terminate automatically upon the termination of the individual's employment at Children's. Termination of employment does not prohibit the individual from seeking medical or allied health professional staff privileges at Children’s as a non-employee in any category for which the individual qualifies. Questions regarding medical or allied health professional staff privileges may be directed to Medical Staff Services.
Preferred:
- Systems management experience and knowledge of health care finance principles.
- Experience and skills that demonstrate strong customer service and organizational abilities.
