Hi, we're Oscar. We're hiring a Bilingual Community Health Worker, Complex Case Management to join our Clinical Care Team team.

About the role:

The Community Health Worker will work with Oscar's Clinical Care Team to help support Oscar members with multiple diagnoses, medication management, and any barriers related to social determinants of health. The Community Health Worker will educate members on improving health outcomes and participate in process improvement and other pilot programs as they arise.

You will report to a Complex Case Management Lead. This is a remote / work-from-home role though field work is required. You must reside in one of the following states: California, Florida, Georgia, or Texas. Note, this list of states is subject to change. #LI-Remote

Responsibilities:

  • Work collaboratively within the diverse team.
  • Develop positive working relationships with Oscar members, providers, case managers, agency representatives, supervisors and office staff from diverse cultural and socio-economic backgrounds.
  • Assist members in their homes, community, or clinic setting communicating the purposes of the programs and the impacts it may have on their wellbeing.
  • Help members identify socio-economic issues that affect their overall health and develop health and social management plans and goals.
  • Document all member encounters and maintain comprehensive electronic member files, which include member notes, release of information, assessments and other medical documents acquired on behalf of the member.
  • Communicate with members by phone or secure messaging using your expertise in an unscripted way to provide education on health conditions and community resources
  • Coach members in the management of their chronic health conditions and self-care under the direction of Case Manager.
  • Assist members in understanding care plans and instructions motivating members to be active and engaged participants in their health and overall wellbeing.
  • Provide support and advocacy during initial medical visits and to assure members' medical needs and referrals are being conveyed.
  • Follow up with both members and providers regarding health/social services plans, where needed.
  • Expand knowledge and use of community resources and services facilitating member access to community resources, including housing, food, clothing, prenatal, parenting, and providers to teach life skills, and mental health services.
  • Help schedule appointments with social services agencies and help complete applications for programs for which they may be eligible.
  • Reduce cultural and socio-economic barriers between members and institutions.

Requirements:

  • CHW certification required from approved Community Health Certification Program, or two years of equivalent work experience
  • 2+ years experience directly related to the responsibilities specified
  • Bilingual in Spanish or Creole
  • High school diploma or GED
  • Ability to travel to facilities/homes within a reasonable distance of hiring address.

Bonus points:

  • AA or Bachelor's degree in social science or health related field
