We are seeking a Advanced Practice Pharmacist Ambulatory South Bay Community who will assess the patient, adjust medications, screen for drug interactions and adverse effects, provide disease/medication education, and explore opportunities to improve the effectiveness, appropriateness, safety and economic impact.

In this position you will:

• Be responsible for providing collaborative drug therapy management (CDTM) in Ambulatory Care Clinic in-person telephonically, and by e-visits
• Evaluate medications that are high-cost, high-risk or high-volume and implements strategies to decrease the total cost of health care while maintaining quality
• Provide comprehensive medication review services and implements changes or makes recommendations to health care providers based on evidence based medicine and national guidelines to improve the quality and outcomes to our patients including reducing re-admissions
• Fully documents all aspects of patient care, and completes all required role with billing and administrative purposes
• Exercise judgment on conditions requiring consultation, referral or evaluation by patient’s PCP or other health care professional

Qualifications:

Required qualifications for this position includes:

• PharmD in Pharmacy Residency
• Licensed as a registered Pharmacist in good standing in California State
• Basic Life Support (BLS) certification
• Pharmacy practice in a clinic/clinical setting
• Two (2) years of clinical pharmacist in an ambulatory/in-patient setting
• Willing to travel to practice site locations
• Prefer fluent electronic health record skills; knowledge of Epic
• Computer competency with Microsoft® operating systems and Windows® based applications
• Must have strong technical writing abilities and understand medical terminology
• Knowledge of clinical automated systems
• Must be able to demonstrate competence in team working capabilities of complex projects involving both medical and informatics along with a variety of professional and administrative individuals
• Must possess the ability to translate end-user needs and regulatory requirements into technical requirements and specifications
• Able to work well on an independent basis and as a team member needing little day-to-day direction
• Emotional demands: Able to work well under stressful situations

Preferred qualifications for this position include:

• Board certified pharmacotherapy specialist or licensed by the State Board of Pharmacy as an Advanced Practice Pharmacist
