Swedish Medical Group is looking for a Senior Program Manager - Network Performance (1.0 FTE, Days) to work in the Collaborative Health Action Team in Seattle, WA.

We are seekin a Senior Program Manager - Network Performance who will work with hospital, transitional and community based care teams to develop, implement, and manage acute episodes of care processes and network(s). Will serve as operational/ administrative lead for this work; which includes preparing and delivering relevant communications to organizational leadership, care teams, community partners, and payers.

In this position you will:

  • Understand, support, and implement the mission of SMG to deliver health care to those in need in the performance of all job responsibilities. Ensure compliance with policies and procedures including the recording of community benefit services and costs.
  • With Administrative Director, Health Services/ VBC, prepare and recommend program budgets, goals, and objectives taking into consideration input from key stakeholders.
  • Participate on SMG committees related to Value Based Care populations and network-wide operational development and strategic initiatives.
  • Develop, recommend and implement strategic planning as it relates to the VBC programs, Swedish Health Services, the region, and the SMG network
  • Create, recommend and implement new programs to support enhanced performance within VBC
  • Formulate plan to meet strategic goals and coordinate with other management
  • Ensure that organizational/regional/program goals are met and that all reporting standards are complied with.
  • Monitor overall performance of the programs - budget, outcomes, financial targets, customer service, etc. - and make changes as appropriate
  • Supervise direct reports providing selection, training, disciplining, terminating, and performance management. Mentor direct reports on issues arising from day-to-day operations
  • Demonstrate competency with all position related software programs
  • Consistent use of appropriate tactics for excellent service.
  • Maintain current knowledge in the field of population health.
  • Perform other duties as assigned.

Qualifications:

Required qualifications for this position include:

  • Bachelor’s degree in health related field or business
  • Five (5) years Increasing responsibility in a hospital or ambulatory clinic setting

Preferred qualifications for this position include:

  • Master's Degree
  • Candidate should have experience with acute episode management strategies and post-acute network development as it relates to VBC populations/ contracts.
