Providence is calling an Ambulatory Pharmacist (1.0 FTE, Variable) to Credena Health Pharmacy Monroe in Monroe, WA.

Perform professional pharmaceutical dispensing functions by providing prescribed medications, drugs, and other pharmaceuticals according to standards and practices for patients in all age groups including pediatric, adolescent, adult and geriatric. Promote safe, cost-effective, rational drug use and provide expert pharmaceutical consultation to other healthcare colleagues.

In this position you will:

  • Demonstrate age specific patient care considerations for the population served.
  • Participate in the development, implementation and monitoring of a pharmaceutical care plan to reach a desired outcome for each patient.
  • Promote cost-effective guidelines and individualized drug therapy.
  • Follow written protocols within the Washington State Department of Health regulations to determine appropriate individual therapy.
  • Minimize or prevent the incidences and severity of adverse drug effects.
  • Provide oversight and direction to staff.
  • Provide drug information and education (written or oral) to patients and other caregivers.

Qualifications:

Qualifications - External

Required qualifications for this position include:

  • Bachelor’s degree from accredited school of pharmacy.
  • Licensed as a Registered Pharmacist in the State of Washington.
  • BLS certification must be obtained within one year of hire.

Preferred qualifications for this position include:

  • Doctorate of Pharmacy degree from an accredited school of Pharmacy.
  • Completed ASHP-accredited PGY-1 Residency Program or 3 years of community or ambulatory pharmacy care work experience.
