Providence is calling a Home-Based Acute Care Physical Therapist (Providence At Home Olympia, Days, 1.0 FTE) to PSCS Divisional Services in the Olympia, WA area. The Home-Based Acute Care Physical Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home setting in accordance with established policies while maintaining the highest degree of quality patient care. As a member of the multi-disciplinary team the therapist assumes responsibility for communication of patient-related issues with appropriate team members or other staff and participates in collection and evaluation of data necessary to assess appropriateness of care at home. The Physical Therapist may also be assigned to orient staff and students. The Physical Therapist works with the patient and family/caregivers in the home setting without direct supervision.

In this position you will:

  • Demonstrates competency in providing quality therapy services including assessment, intervention, program planning and implementation, discharge planning, related documentation and communication. Also demonstrates competency with identifying and making appropriate referrals regarding medical, social and emotional factors influencing care.
  • Evaluates physical and functional status as well as cognitive, psychological and perceptual function to determine the client's capabilities to function safely at home. The home and family/caregiver are assessed for safety risks, and ability to support and care for the client at home.
  • Develops a therapy plan of care in collaboration with clients/families. The plan of care is communicated with the multidisciplinary team to form a collaborative client care plan.
  • Provides care in accordance with physician orders and the established plan of care. Assesses effectiveness of treatment and modifies treatment to achieve goals.
  • Documents all evaluations, treatment and coordination of care contacts in appropriate facility/agency format. Meets agency time guidelines for completing documentation. Documentation reflects good clinical documentation as well as meeting agency billing and reimbursement requirements.
  • Produces expected results given present skills, knowledge, abilities, training and education for the following age groups: adult and geriatric.
  • Demonstrates an ability to treat the wide range of diagnoses and be knowledgeable in therapy treatments, goals and contraindications for treatment. Treatments are adapted to the unique home setting and to the client and/or caregiver's ability to follow through safely and correctly. Requests consultation, supervision, or continuing education to learn specialized techniques or treatment approaches.
  • Provides client/family education and counseling regarding issues related to treatments and goals, safety risks and/or other issues within scope of practice. Teaches home exercise programs and compensatory techniques or skills for managing ongoing physical or functional deficits in the home.
  • Communicates pertinent medical and clinical information (medical, social, and emotional factors) to the Interdisciplinary team, including information regarding the patient’s care plan and implementation of changes per physician order. Also communicates pertinent information to team members, supervisor, staff and external sources as appropriate.
  • Provides supervision for physical therapy assistants and students per regulatory guidelines. Participates in orientation of new staff members by orienting them to the unique clinical practice of their discipline in the hospital at home setting.
  • Plans for discharge. Participates in team reassessments and discharge planning. Teaches client or family discharge therapy program. Recommends follow-up rehabilitation needs.
  • Evaluates, recommends and assists in obtaining adaptive equipment. Fabricates adaptive equipment as needed. Adapts equipment and techniques for use to the home setting, ensuring optimum function and safety. Provides consultation or makes appropriate referrals regarding resources and barriers in the community.
  • Independently manages own case load, plans and schedules home visits coordinating with other team members as needed to maximize effectiveness of overall care plan. Organizes weekly schedule to attend required meetings within schedule of client care needs. Responds to urgent client needs by prioritizing and triaging treatments. Communicates schedule and work plan to scheduling coordinator, supervisor and other team members as appropriate.
  • Organizes and coordinates transfer of client information when additional complementary therapies are requested and when client care is provided by another clinical facility/agency.
  • Educates/consults with hospital at home staff, hospital staff and community resources regarding therapy services in the hospital at home setting. Demonstrates a working knowledge of all available hospital at home services in order to make appropriate referrals for those services. Provides in-services to staff regarding new treatment/techniques learned.
  • Provides consultation regarding Physical Therapy services to other disciplines, as needed or as requested.
  • Instructs and supervises home health aide programs to be carried out by the aide including functional skill techniques, exercise and activity programs needed to achieve hospice goals. Instructs others in the techniques in order to reach goals.
  • Participates in program planning, gives input to program policy discussions particularly in those areas impacting patient care.
  • Participates in in-service programs and community continuing education needed to remain clinically current in evaluation and treatment techniques.
  • Meets agency productivity standards established for therapy. Uses time efficiently and effectively.
  • Attends required team meetings. Participates in agency committees as assigned by supervisor. Participate in facility/agency program planning, gives input to program policy discussions particularly in those areas impacting client care.
  • Participates in the orientation of new facility/agency staff members by orienting them to the unique clinical practice of their discipline in the home setting.

Qualifications:

Required qualifications for this position include:

  • Education sufficient for licensure
  • 1 year of therapy experience with adult geriatric and/or individuals with physical disabilities
  • Current Washington State Physical Therapist License
  • Valid Driver’s license
  • Current CPR certification required within 30 days of hire.
  • Requires access to daily use of an insured reliable vehicle or an alternate ability to travel to sites as required.

Preferred qualifications for the position include:

  • 1 year of therapy experience in home care or in related rehabilitation setting
  • Experience with the geriatric population
