HARBORVIEW MEDICAL CENTER PHARMACY has an outstanding opportunity for a FULL-TIME CLINICAL STAFF PHARMACIST - (PCAP NEUROLOGY/REHABILITATION MEDICINE) (Day Shifts) . The Clinical Staff Pharmacist acts as an integral member of the Neurology and Rehab Medicine teams to provide pharmacotherapeutic recommendations to a multidisciplinary team, optimizing patient care and improving patient outcomes. These responsibilities include but are not limited to clinical duties, drug distribution, teaching and research, and management as it pertains to administrative aspects of the care of Neurology and Rehab Medicine patients.
RESPONSIBILITIES:
Management:
• Develop and maintain policies and procedures for the Neurology and Rehab Medicine services.
• Initiate and maintain lines of communication between nurses, pharmacists, physicians, and other care providers on behalf of Neurology and Rehab Medicine patients.
• Assist in the development and carrying out of quality assurance programs for the Neurology and Rehab Medicine service, including ensuring organizational compliance with national practice standards (e.g. CORE measures, UHC benchmarks, etc)
Clinical:
• Participate as an active member of the Neurology and Rehab Medicine team to assist in achievement of specific therapeutic outcomes by continuous monitoring of drug therapy.
• Participate in medication order review, clarification, validation, and order verification.
• Monitor the nutritional needs of Neurology and Rehab Medicine patients; review and write TPN orders.
• Provide drug information and inservice education to pharmacy staff, nurses, residents and other providers on critical care-related issues.
• Facilitate communication with other providers upon patient transfer to other patient care areas.
• Obtain medication histories and perform medication teaching with patients or caretakers.
Distribution:
• Develop and maintain procedures to improve the speed and accuracy of medication distribution to all patients.
• Evaluate and enter inpatient orders and respond to telephone inquiries from professional colleagues.
• Support inpatient pharmacy operations.
Academic:
• Precept critical care and general pharmacy practice residents and undergraduate pharmacy students.
• Optional: didactic lectures at the University of Washington School of Pharmacy.
Research:
• Participate in collaborative research with the University of Washington Schools of Pharmacy, Medicine, Nursing, and other health professionals.
• Encourage pharmacy staff and residents’ understanding of and participation in practice-oriented critical care research.
• Publish and encourage publication of scholarly articles in professional journals.
• Participating in collaborative research projects with other health professionals to improve patient care.
Other:
• As assigned. REQUIREMENTS: Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
LEGAL REQUIREMENT: State of Washington Pharmacist License.
