Crisis Integration Specialist I - Posting #21645
Position Summary:
Sea Mar Community Health Centers is seeking a Full-time Crisis Integration Specialist for our Behavioral Health Program in Everett. The Crisis Integration Specialist provides engagement and intensive case management services to individuals who are high utilizers of emergency services, criminal justice system and community social services. The Crisis Integration Specialist provides outreach, long-term assertive engagement and supportive services for participants through intensive case management activities and collaboration with community partners, service providers, housing providers and other community organizations.
In addition, the Crisis Integration Specialist provides the development of a client-centered action plan as well as the development of a Community Support Plan. Assistance to the Director in program development, policy and procedure development, compliance with WACS and RCWS, and other administrative tasks performed are as directed. This person will be required to follow other instructions and to perform other duties as assigned by the supervisor.
