Our goal is partnering with the nation’s leading health systems in their strategic evolution to positively impact and improve the healthcare delivery system. As a member of a team of nurses, social workers, registered dieticians, physicians, pharmacists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people living with multiple chronic illnesses. You will link with your patients on the phone and through email, in the patient’s home, hospital, and in the physician’s office - essentially however and wherever the patient needs your assistance to improve their health, better comprehend their illness and coordinate their care.

Responsibilities

• Schedule and coordinate communication for the team, physicians and patients, e.g., setting up conference calls, logistics of medication therapy, and patient transportation.
• Function as the central point of communication for all members of the population health team, their patients, and their patient’s physicians.
• Coordinate patient care, in conjunction with other members of the population health team, by serving as the contact point, advocate and resource for the patient, their family and their physician, building effective relationships through reliance and communication.
• Provide excellent customer service to patients and physicians.
• Maintain and manage the team’s databases.
• Provide additional administrative support as needed.
• Review medical records and update the database
• Assign cases to appropriate team members

Qualifications

Required

• Prior administrative experience required
• Proficiency with the Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
• Passion for top-notch customer service.
• Ability to work independently and solve problems in a fast-paced, quickly changing environment.
• Strong organizational and prioritization skills.
• Exceptional communication skills, both written and oral.

Preferred

• Associate’s degree preferred.
• Experience as MA, respiratory therapist, EMT, paramedic or register dietician preferred
• Working in healthcare a plus, but not required.

Technical requirements:

During the current pandemic Evolent employees are working remotely from home. As such we require that all employees have the following technical capability at their home: High speed internet over 10 MBPS and, specifically for all call center employees, the ability to plug in directly to the home internet router. These at-home technical requirements are subject to change with any scheduled re-opening of our office locations.
