Transforming care requires strong leadership, deep expertise in multidisciplinary clinical practice, enabling and intuitive technologies, advanced data analytics, and visualization and systems modelling to derive intelligent clinical and business solutions and effective multi-channel communications.

Philips Population Health has these capabilities and offers them in the form of design and implementation of Tailored Ambulatory Care Programs supported by the creation of an Enterprise Telehealth Centre where clinicians are equipped to deliver virtual care to patients regardless of their physical location. Enabled by patient monitoring solutions, on-demand audio/visual access, and powerful clinical decision support tools, these clinicians deliver a significant improvement to patient treatment outcomes and reduced provider costs.

Your Opportunity:

We have an opportunity for a highly versatile, energetic, courageous, and passionate individual capable of influencing the creation of new virtual models of care for our hospital system partners. You will become a key member of the ANZ Philips Population Health team located in either Sydney / Brisbane as a Clinical Transformation Consultant for our Ambulatory Care business. You will play a critical role in executing Philips Population Health mission by supporting local partner provider team members to transform their practices.

Reporting to Clinical Transformation Manager - Ambulatory Population Health you will work closely with the PHM team, development partners, global teams, and suppliers. You will be a key member of the ANZ Population Health team as a Clinical Transformation Consultant.

Using your exceptional customer relations and change management skills, you will play a key role in ensuring that our Ambulatory care solution is adopted and implemented throughout the full lifecycle of the relationship with our provider partners. This process begins with pre-sale presentations/demonstrations and moves through to localization, installation, and evaluation, ensuring continuous improvement.

Together with our partners, you will ensure that there is appropriate clinical governance and support the on-going maturity of these models of care to improve health and economic outcomes. Your primary goal is demonstrating growth in patient numbers, positive partner experience, and evidence of efficient and effective practice change resulting in the improved value for the business through demonstrated KPI’s for each provider partner.

Other key responsibilities include:

  • Acting as a powerful change agent for clinical transformation - embedding new clinical practice processes at the client site.
  • Mapping current patient care workflows to future virtual care inspired workflows
  • Periodic review of clinical practices and dissemination of quality and efficiency reports.
  • Drive the integration and adoption of behavior change methodologies to become standard practice.
  • Collaborating with Philips Program Executive to produce effective relationships with hospital executive team
  • Providing educational and training sessions.
  • Travel throughout Australia to meet with health service providers - supporting demonstrations and clinical practice transformation.
  • Coordinate the configuration, set up and / or modification of the clinical system platform following provider partner protocols.
  • Compose, submit, and track clinical system questions and configuration requests to correct identified systemic clinical operations issues.
  • Prioritize issues identified by business partners, internal team members, and or partner representatives and monitors progress in the resolution of these issues

We are looking for

  • 5+ years of clinical health services experience
  • Demonstrated experience in practice re-design including for example: (Practice Transformation; Training development and delivery; Quality Improvement; Chronic disease service development or other relevant experience).
  • Experience in leading clinical teams or projects
  • Experience in formal presentations and/or clinical training
  • Understanding of workflow processes and development
  • Understanding the change process
  • Technological capability to the extent you will become highly proficient in the use, demonstration, and training of Philips solutions and inform product enhancement over time.
  • Excellent interpersonal/communication/collaboration skills
