Overview

Fairview Pharmacy Services is looking for an experienced Oncology Pharmacist to provide clinical support and consultative services in our complex clinic based pharmacy operation. This position partners with key stakeholders to drive medication safety, customer service, process improvement, clinical and formulary programs and initiatives and regulatory compliance. The Oncology Pharmacist will act as a primary resource in the development and education of students, staff, and interdisciplinary members of the care team.

This role will provide clinical oral chemotherapy services to the St. Joseph, Woodwinds, and St. John's Hospital Outpatient Oncology locations, working out of St. John's Hospital on a rotational basis. When not rotating through oral chemotherapy at the assigned locations, this pharmacist will be providing support to the outpatient oncology clinic and outpatient oncology and non-oncology infusion area at the MHealth Fairview Clinics and Surgery Center.

Responsibilities/Job Description

  • Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
  • Act as resource for pharmaceutical care to patients This includes partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives.
  • Provide clinical pharmacy services to all patients in assigned area including completing review for all patients daily, monitoring, adjusting patient therapies and documenting changes according to department policies.
  • Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.
  • Effectively consult and educate patients, staff and students on drug
  • Execute the function of staff pharmacist.

Qualifications

This position requires completion of an ASHP-accredited PGY1 residency program or one year of pharmacist experience in a complex pharmacy operation. The ideal candidate will have 3 years of experience in a specialty setting and board certification as an Oncology Pharmacist.
