Responsibilities & Requirements

In the Care Advocate role, you'll provide telephonic, e-mail and/or community forums to outreach to members in an integrated care management model to reduce modifiable risks factors, encourage program engagement and participation through education, lifestyle management, and referrals to other internal services and resources. The end result is to achieve personal health outcomes by providing support, encouragement, and education to improve well-being and prevent/minimize disease.

  • The Care Advocate II would have a Bachelor’s degree in health promotion, health education, nursing, athletic training, nutrition, counseling or other related field and at least 3 years of Care Advocate experience or equivalent combination of education and experience
  • Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, pre-post-natal care, stress reduction and chronic conditions
  • Chemical dependence treatment experience is preferred.
  • Initiate outbound telephone calls to members in potentially complex healthcare situations (e.g. hospital discharge or admission to skilled nursing facility).
  • Provide members with support, encouragement and education to improve the member’s ability to make informed healthcare decisions, navigate care options and to take actions in order to improve well-being and prevent/minimize disease.
  • Strong communication skills with the ability to effectively interact with a wide variety of individuals
  • Track record of customer service excellence
  • Organization skills and proven ability to effectively handle multiple tasks and prioritize workload
  • Knowledge of technology and applications such as Excel, Word and PowerPoint
  • Understanding of care management principles
  • Identify areas in need of improvement in all aspects of program and provide recommendations for solutions
  • Ability to flex hours to staff early evening calls
