We are currently looking for a Pharmacist with Hospice experience for our Evergreen Professional Center Pharmacy.
Job Summary:
The pharmacist monitors drug therapy; fills and verifies prescriptions; provides drug information and counseling to patients. Provides pharmaceutical consultation to physicians, nurses, patients and the general public. Compounds and dispenses all pharmaceuticals from the Evergreen Professional Center (EPC) Pharmacy.
Essential Job Functions:
1. Performs basic pharmaceutical care review for each patient.
2. Responsible for general clinic duties, is a resource to staff, providers and patients, plans and develops goals, promotes quality improvement processes. (see General Clinical responsibilities)
3. Responsible for dispensing medication and distribution system for tracking, adhering to all State and Federal pharmacy regulations.
4. Performs other duties as assigned.
General Clinical Responsibilities:
• Provides drug information to healthcare providers in reply to specific requests in a timely fashion and presents the information in a form that aids the decision-making process for a specific patient where applicable.
• Provides in-service training to healthcare providers as requested.
• Maintains patient confidentiality.
• Participates in regular educational offerings of the pharmacy and/or hospital.
• Reviews quality improvement (QI) memos regarding medication related incidents in area of practice, provides required follow-up and documentation.
• Demonstrates clinical expertise and understanding of the needs and behaviors of all special patient age groups.
• Assists in the training of pharmacists, pharmacy interns, pharmacy technicians, students, and volunteers.
• U nderstands the role of pharmacist and technician and the duties assigned to each.
License, Certification, Education or Experience:
REQUIRED for the position:
• Washington state pharmacist license
• Hospice experience preferred
• Math skills necessary to perform drug dosing and compounding calculations
• Current Healthcare Provider BLS certification by date of hire
• Completion of initial prescriptive authority protocol training and/or evaluation.
• Demonstrates appropriate pain assessment and care techniques.
