Who You’ll Be Working With:

The Community Health Worker will work as an extension of the clinical care team and specifically under the guidance of an RN or Social Work Care Manager. The individual taking this role will be assigned patients and will manage his/her caseload through in-person, telephonic and electronic communications and coordination.

What You’ll Be Doing:

  • Work under the guidance of the Senior Director of Clinical Operations or Manager of Clinical Operations
  • Conduct initial outreach calls to encourage patients and care givers to participate in care management programs
  • Document their activities in the Identifi platform, including care plan activities conducted
  • Engage with patients who need assistance with self-care needs
    • Address language and cultural barriers to care and self-care
    • Coach and guide the patient to meet both personal and clinical goals
  • Schedule provider visits on behalf of their patients
  • Remind patients of and/or accompany patients to their visits
  • Help patient access community and governmental based service agencies including assistance with completing applications/paperwork
  • Reinforce the symptom response plans developed and taught by the provider
  • Arrange community resources that address social determinants of health
  • Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
  • Participate in the integrated care team meetings
  • Assist with process and supportive tasks based on market need, such as completing provider notifications of patient admissions and discharges, and verifying patient contact information for outreach
  • Collaborate with other care team members to ensure best patient interventions and outcomes

The Experience You’ll Need (Required):

  • Medical Assistant, Certified Nursing Assistant, Engagement Specialist, or Community Health Worker Certification
  • Knowledge of San Antonio, TX communities and ability to provide own transportation to and from surrounding areas as needed

Finishing Touches (Preferred):

  • Experience working with patients with chronic and behavioral health needs
  • Ability to connect with people and understand the challenges they face
  • Demonstrated success in working as part of an interdisciplinary team including communicating and working with Physicians, Registered Nurses, Social Workers, Pharmacists, and, Health Coaches, and Dietitians
