Overview

The Registered Nurse bears full responsibility for the quality of health care he/she provides to the residents at Seven Hills at Groton. Provides quality care to children and young adults in accordance of the Nurse Practice Act of Massachusetts.

Responsibilities

Conveys through nursing practice a clear sense of the role and identity.Assumes supervisor of the shift duties as needed after being oriented to the position.Contributes to individualized plan of care in which needs are identified and achievable goals are set.Implements nursing care plan in an organized manner; establishes and reorders priorities as needed. Evaluates outcomes of basic nursing interventions. Incorporates the prescribed medical regimen into the nursing plan of care.Delivers care that is consistently reflective of the standards of care at Seven Hills at Groton.Administers medications and treatments safely according to established nursing practice.Systematically assesses the resident’s basic health status, records and related health data.Recognizes changes or trends in patient status.Participates in analyzing and interpreting data and making informed judgments as to the specific elements of nursing care, using appropriate resources and NP/MD consultation.Participates in the health teaching required by the family and other responsible parties.Collaborates, cooperates and communicates with other health care providers to ensure quality and continuity of care.Delegates responsibilities for the performance of selected resident care activities to assistive personnel within the established delegation framework and provides appropriate supervision.Ensures that all nursing personnel assigned to RN comply with the written policies and procedures established by this facility.Meets with assigned nursing staff, as well as support personnel, in planning the shift’s services, programs and activities. Ensures that assigned certified nursing assistants (C.N.A.’s) are aware of the resident care plans. Ensures that the C.N.A.’s refer to the resident’s care planprior to administering daily care to the resident.Makes a written and oral report/recommendation concerning the activities of you shift as required.Admits, transfers and discharges residents as required.Completes accident/incident reports as necessary. Appropriately notifies and informs supervisory staff.Performs administrative duties such as completing 30 day summaries and edits as necessary.Encourages attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies.Receives telephone orders from physicians and record on the Physician’s Order Form.Accurately transcribes physician’s orders to resident charts, Kardex, medication cards, treatment/care plans, as required.Charts nurse’s notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Report all discrepancies noted concerning physician’s orders, diet, charting error, etc., to the Nursing Supervisor.Signs and dates all entries made in the resident’s medical record.Ensures that an adequate supply of medications is on hand to meet the nursing needs of the residents. Properly orders medications in a timely fashion per pharmacy guidelines.Ensures that the narcotic records are accurate for your shift. Notify the Nurse Supervisor of All drug and narcotic discrepancies noted on your shift.Reviews MAR for completeness of information, accuracy in the transcription of the physician’s order and necessary initials to indicate doses administered.Disposes of drugs and narcotics as required and in accordance with established procedures.Responds to complaints and grievances made by family or responsible parties. Report to nursing supervisor as necessary. Follows Seven Hills at Groton grievance policy.Participates in the orientation of new residents/family members to the facility.Makes rounds with physicians and actively participate in planning care for assigned residents.Arranges for diagnostic and therapeutic services, as ordered by the physician, and in accor

Qualifications

Licensed as a Registered Nurse in Massachusetts. A background in developmental disabilities and/or pediatrics is preferred.
