    Responsibilities

    The goal of the Care Navigator is to ensure delivery of optimal care for the patient while promoting fiscal responsibility.

    Ensures safe, efficient, cost-effective care by assessing the patient’s clinical, psychosocial, and financial parameters. Develops and implements a patient-centered care plan for the acute health episode that begins with the index hospitalization. Collaborates with the patient, patient representatives, and interdisciplinary team to facilitate optimal outcomes as the patient transitions through the healthcare continuum.

    Works in a manner which supports the mission and vision of UW Medicine Northwest Hospital. Ensures that daily operations, procedures and standards of care are maintained in accordance with hospital and The Joint Commission guidelines.

    Qualifications

    Education Background

    • Bachelor of Science in Nursing required. At the discretion of the manager, additional years of relevant experience may be substituted for the degree. Master’s degree preferred.

    Licensure/Certification

    • Registered Nurse with current Washington State license.
    • Case Management certification preferred.

    Work Experience

    • Minimum of five years of clinical practice in an acute care setting within the last five years is required.
    • Three years previous experience in care management is preferred.
    • Knowledge of medical screening criteria is preferred.

    Special Training/Skills

    • Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit(s)/department.
    • Articulates with written & verbal communication in English with all stakeholders (patients, staff members, clinicians, practitioners, and the general public).
    • Excellent negotiation, presentation, and collaboration skills.
    • Communicates professionally as an expert and team player simultaneously.
    • Ability to problem-solve by evaluating options, generating and clearly communicating solutions.
    • Possesses a strong commitment to a positive team environment with the ability to contribute expertise regarding transition plan and follow leadership directives when appropriate.
    • Demonstrates personal accountability within a larger team.
    • Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit/department.
    • Ability to flex to assist other department members as needed.
    • Ability to grasp new ideas and integrate them into desired results.
    • Adapts and prioritizes in the setting of fluctuating work load.
    • Computer skills consistent with electronic charting and referral system

    Physical Abilities

    Physical demand Level: Sedentary

    In an 8-hour workday, must be able to-

    Lift / Carry 10 pounds occasionally
